PAST 15 MONTHS REFLECTIONS

I share health status updates about mobility, nutrition and several recently diagnosed cardiovascular issues.

MOBILITY

MORNING & EVENING SESSIONS

Twenty seven months have passed since my upper torso was subjected to significant soft tissue trauma during a car accident.

 SLOWLY  RECOVERING

During this time I have regained abilities to engage in many whole body movements involving co-ordination of both arms and shoulders including GOMUKHASANA (clasping of hands behind my back) - as demonstrated in a photo from a previous post on March 23, 2022, the one year anniversary of the incident.

In the past 15 months, yoga props (exercise ball, block, strap, sandbag, wrist and knee supports among others) have continued to enhance my whole body movements (= achieve deeper range of motions) during each of the two daily sessions.


NOTABLE IMPOVEMENTS FROM MASSAGE

Monthly scheduled bodywork TUI NA sessions with RAY HOYT have contributed to an ongoing recovery during the past 12 months in addition to the props I use daily available in my condo yoga studio.

MOBILITY CHALLENGES REMAIN

DUPUYTREN'S CONTRACTURE

The progressive connective tissue disease I had been experiencing and only diagnosed at 65 - independent from the car crash - continues to impact my yoga activities and requires daily attention by controlled, gentle stretching. 

I have continued to devote between 30 to 60 minutes each evening to safely open (= lengthening muscles & connective tissues) in both hands (palms, fingers) before starting my sequence of asanas.

LOSS OF MUSCLE MASS (SARCOPENIA)

The most notable and lingering obstacle to my recovery after being injured is the loss of significant muscle strength in whole body movements involving the use of one and two hands to support my full weight.

Immediately after the accident, I was unable to execute the EIGHT ANGLES asana - making the transition from a sitting position into a weight bearing posture with a lifted chest and elevated pelvic area with both legs extended laterally as demonstrated in the photo below.

An asana once done relatively well and often accompanied by an ability to do pushups for over a decade was beyond my abilities after to the extensive injuries.

During the past 15 months, my recovery has progressed slowly by using my total body weight alone as the primary source of resistance for brief durations to acquire muscle strength.

AN UPDATE

"LESS IS MORE"

MODIFYING MY STRATEGY

Listening to my body, carefully assessing daily asana movements and outcomes and making modifications to avoid muscle and connective tissue injuries led to a different and more effective approach during 2023.


I focused on the seven weight bearing asanas listed below to gradually develop upper body strength.

> During the past six months I have included one of the seven asanas listed above and added it to my customized sequence of movements during daily evening sessions. 

> The following night a different asana from this list is selected and continues for the week before the cycle is repeated. 


PROGRESS HAS BEEN EXPERIENCED

These changes, accompanied by making modifications to the number repetitions and/or the duration holding a posture, have led to improved upper body strength for each of these asanas including EIGHT ANGLES.

ACKNOWLEDGING OBSTACLES

I do understand the ability to do a specific asana with facility and range of motions achieved in the past may not be possible in the future.

My yoga instructor Ray Hoyt often reminded the classes he taught to appreciate what one can do now, that ones yoga practice can be a humbling experience.

MY DAILY WALKING

I have walked every day during 2023 - extending an activity without interruption for over two years.

Earlier this Spring I began to use the stick as a light weight - an effective yet gentle means to engage compromised muscles and connective tissues of both hands and upper torso while walking for durations lasting +/- 90 minutes.

BACKGROUND

> My daily nutrition choices required prompt changes when, in 2021, I was diagnosed with CORONARY ARTERY DISEASE - a serious and previous unknown condition.


> Assessment of imaging data led to a CALCIUM SCORE indicating I was at a high statistical risk for a cardiac event during the following five years; as a consequence, three significant actions were initiated:

(1)

 DECREASING HYPERLIPIDEMIA

I began taking daily Rosuvastatin (40 mg) and an Aspirin (81 mg)

&

My lipid panel values from July 2022 continued to improve from 2021; my updated 2023 lipid assay numbers will be obtained during August.

 ENHANCING MORNING MEALS

AT

THE ROSE ROOM 

(WEBSTER, MA)

 I have continued to consume their seasonal and diverse meals whose ingredients are locally sourced and freshly prepared.


Staff preparation on-site of breads (biscuit, molasses buns, English muffins and wheat flatbread) are enjoyed on a regular basis while selected pastry on infrequent occasions - acutely aware of my coronary atherosclerosis status.

Both water and hot coffee are consumed daily.

ONE NOTABLE CHANGE

In August, 2022, I began to add various raw veggies, fresh and dried fruits, legumes, mushrooms, nuts and seeds from home to my purchased selections.

This practice was begun with the knowledge of THE ROSE ROOM owner JESS SABINE and continues to this day.

I intentionally have consumed less food at this time of the day.

Some small combination of veggies, nuts, seeds, fruit and granola are eaten during afternoon hours - the only other source of nutrition during each day.

I have continued a practice of several years of not eating any meats; no obvious negative health consequences have been experienced based on blood, urine lab data.

I have continued a practice of several years to limit daily food consumption to an 8 - 9 hour period without any sense of feeling hungry, malnourished or sensation of weakness.

Water continues to be enjoyed throughout the day - acknowledging the importance of remaining hydrated.

An electrocardiogram taken during an annual meeting with my Geriatric Specialist Dr. Faucher last July detected a previous unknown electrical conduction delay condition diagnosed as a LEFT BUNDLE BRANCH BLOCK.


I was referred to Dr. MAZEN ROUMIA, a CARDIOLOGIST at St. Vincent's Hospital in Worcester, MA and underwent three additional cardiovascular evaluations during the past year.

The tests, major imaging observations from each are listed below:

NOTE 

The data from these cardiovascular evaluations during this past year have contributed to a revision of my current health assessment at 72: from relatively good to fair. 


I do not know when these several cardiac conditions, possible future risk factors first developed: most likely they are a reflection of aging - involving likely interactions of my genetics and diverse environmental factors.


A NEW REALITY
These tests provide useful baseline data establishing both electrical and mechanical cardiac issues that currently seemingly appear to be asymptomatic but subject to change.

WITH 
IMPORTANT QUALIFICATIONS

[1] I acknowledge not experiencing any thoracic pain or symptoms of weakness, fatigue during any of my two daily yoga, walking physical activities during past years. 


[2] My daily emphasis on lifestyle choices: yoga mobility, walking and making better nutrition decisions for many years have led to positive, significant and valued health outcomes recently noted and commented on by my Geriatric Specialist and Cardiologist.


FUTURE EVALUATIONS

> These two newly revealed heart issues will continue to be monitored as will the status of my coronary artery disease (atherosclerosis).

> I have a scheduled meeting with the Cardiologist this December that will include further conversations about whether possibly obtaining more detailed coronary artery imaging via a catheterization procedure is warranted.


Remaining focused on  my whole body mobility and nutrition will continue to be a high priority.

A YEAR LATER: REGAINED MOBILITY FOLLOWING INJURIES

I share updates on my overall health status including yoga mobility and nutrition since experiencing extensive upper body muscle and connective tissue trauma on March 23, 2021, days before my 70th birthday.

I took the above asana image on March 16, 2022 using a smartphone camera ten second exposure delay after a recent evening yoga session.

NOTABLY

These movements demonstrate a gradually acquired range of motions involving many upper body muscles (including chest, shoulders, back) and associated connective tissues/joints not possible a year ago.

&

With bent arms and elbows brought towards vertical, I am now able to easily clasp fingers deeply without experiencing any discomfort or pain.
____

Being able to move into GOMUKHASANA again is evidence of a relatively good range of motions for a senior approaching 71. 

Twelve months ago I recall being unable to hold and barely move a toothbrush without considerable pain, also unable to extend my arms forward in a restorative CHILD'S pose.

My Geriatric Specialist, Dr. Jarrod Faucher, will undoubtedly be pleased with my current shoulder mobility status.

Being able to execute this challenging asana in less than ten seconds requires:
  • both palms and fingers of each hand to be mobile to avoid serious injury from a diagnosed progressive and compromising connective tissue disease (Dupuytren's Contracture), both wrists need to be mobile as well
  • mobile arms, shoulder girdle, back and chest muscles, associated connective tissues and joints
  • coordinated mobility of pelvic girdle, leg, back and spinal muscles, joints

These conditions have led me to devote time (currently 45 - 60 minutes) to a warm up period each evening before starting my customized whole body sequence of asanas.

On many occasions I have altered planned asanas in response to how my body was moving during this time.

NOTE

In the FIREFLY VARIATION asana shown above, I experienced a restricted mobility of my lower back and pelvic girdle that occurred during the evening session and limited the upper elevation of my legs.

On other occasions during these past several months, I have been able to easily move elevated legs up higher and towards my torso. 

To avoid overuse and encourage time for recovery, I have limited my FIREFLY VARIATION to every other day.

My mobility during these past three months has benefitted from using an exercise ball before both morning and evening yoga sessions, after walking and at various times during each day to lengthen spine, back, pelvic areas and associated muscles compromised from sitting, also while reading, writing and other activities.


The exercise ball enabled me to safely, gradually initially open (lengthen muscles) of the back, pelvic and shoulder girdles and eventually my whole body.

Being able to have my feet and head on the mat simultaneously without any discomfort has been a welcomed outcome.

ALSO

Using this prop before retiring each evening has contributed to experiencing less muscle issues at night. 

As a consequence of starting this practice, I have been better prepared for the whole body morning yoga sequence of asanas.

My monthly meetings with RAY HOYT during the past three months have contributed significantly to healing of muscles and connective tissues - especially of the upper right arm, shoulder.

Our bodywork sessions have focused on (1) issues identified with specific sources of discomfort, pain [for example, in my upper right arm biceps, deltoids, rotator cuff infraspinatus, supraspinatus muscles] and (2) more general maintenance areas (lower, mid, upper back, neck, lower legs, calves) 

At this time my upper right arm biceps and deltoids continue to be monitored daily - fortunately I am experiencing pain on fewer occasions during the past three months.

(LOSS OF MUSCLE MASS, STRENGTH)



Despite frequent challenging winter conditions, I have walked every day since January 1, 2022 - successfully continuing this still enjoyable physical activity from the previous year.

Continuous walking, often for +/- 90 minutes, has enhanced my cardiovascular system and breathing, likely moderating blood pressure systolic, diastolic values as well.


Significantly, daily walking has provided an effective way to help maintain muscle mass, tone and strength to counter and possibly slow the rate of sarcopenia (think = muscle atrophy) often documented with higher frequencies among older, more senior individuals.

&

At this time I am able to walk without experiencing any pain in my arms, neck, shoulder and chest areas - further evidence of my upper body healing from soft tissue trauma.

I have begun to read older and more current research papers about our current understanding of the molecular and cellular basis of sarcopenia and will share some thoughts on this topic in future blogs.

BETTER NUTRITION

SINCE LAST DECEMBER


My lifestyle focus on making more informed nutrition choices have continued to contribute to positive health outcomes since January.

Both (1) calorie restriction and (2) time-restricted eating habits have continued to inform my lifestyle approaches to daily nutrition decisions.

___

Consuming more nutrient-rich and mostly plant-based food sources containing proteins, carbohydrates and fats has been a daily goal, a way to avoid muscle mass loss.

A high priority has been focused on avoiding highly processed products with elevated concentrations of salt, sugar and fat.

&

As a consequence of remaining focused and disciplined daily, I have been able to achieve (1) a healthy nutrition status resulting in (2) a leaner and more mobile body (3) while maintaining a desired adolescent, high school weight.


NOTEWORTHY

I have not experienced any sense of fatigue or of being malnourished following my current daily eating practices - currently being able to engage in all daily normal and physical activities (including yoga, walking) of interest without any difficulties.

&

Likewise, I have not experienced a craving for any foods - especially 'snack, bakery' selections (including cookies, brownies, biscotti, cake, pies).

I have intentionally mostly eliminated these and other processed items in response to a diagnosis of coronary artherosclerosis, being at a high statistical risk for a negative coronary event.

Consuming more water has been a priority these past months.

NOTABLE 2021 GOALS ACHIEVED

I was able to achieve mobility, nutrition, weight and wellness objectives during the past twelve months while confronted with significant and lingering upper body traumas.

Addressing (1) limitations from a right shoulder impingement and referred upper arm pain (pre Jan - Mar) and (2) more extensive and painful upper body trauma on March 23rd has been a challenging, lengthy healing process throughout this year.

Chest, back, neck, shoulders, arms, wrists and hands were all initially negatively impacted and extensively limited my range of motions from a car accident.

  -

The Downdog asana shown above was one of many hatha yoga movements I used to evaluate my mobility status.

Being patient and focusing on what was possible, what I could safely do each day was rewarded with a gradual return of range of motions before both injuries.

It has been rewarding - both a source of satisfaction and motivation - to successfully complete another full year of morning and evening yoga sessions in my condo studio while confronted with injuries not experienced during the past decade.

I was able to assess what asanas were possible, to make modifications as deemed necessary to accommodate limitations and to make changes to the sequence of asanas as deemed necessary while recovering.

My FIREFLY & EIGHT ANGLES asanas have not been done for several weeks since my last massage session to allow for chest, arm and shoulder recovery to occur without introducing new stress despite being able to do both of these movements.


Documentation of every morning and evening session provided instructive, useful information I intend to share with others in the future.

I exceeded initial expectations in January with my walking this year - remaining focused, disciplined despite diverse weather conditions including the winter bitter cold, summer withering heat and humidity.

-

Especially satisfying was my success dealing with low temperatures, often high winds along with snow, sleet and icy and slippery surface conditions: none of these were considered a determent from being outside.

Likewise, early walks begun after 4am - starting in the dark at times during the summer - avoided the highest elevated afternoon temperatures and humidity.

Throughout the year I was successful to deal with most prevailing conditions except heavy rains. Keeping hydrated was a priority.

2022 PLANS

New walking shoes and rain gear to avoid getting soaked will be purchased. I have begun to evaluate available jacket options that provide protection but can breathe.

ALSO

Adding some form of biking along with walking entering the new year to complement my daily yoga activities is being considered.

I am looking forward to my walks on Wachusett Mountain, at the Tower Hill Botanic Garden and possibly joining The Massachusetts Walking Tour in Western MA with musicians Mark Mandeville and Raianne Richards among other locations.

EATING BETTER  

NUTRITION

Data obtained from my coronary calcium imaging, calcium score indicating high risk atherosclerosis from hyperlipidemia led me to begin taking a statin and aspirin daily.

This disclosure resulted in changes to my daily nutrition choices and to identifying alternative food sources to lessen the consumption of components (elevated and addictive levels of salt, sugar, fat) that would likely contribute to negative health outcomes.

HIGH NUTRIENT SOURCES

ARCADE SNACKS in Auburn, MA, has continued to provide me with diverse nuts, seeds and dried fruits - on most days, over twelve items are mixed and contribute to my first early morning meal.

Vegetables, fruits, legumes, mushrooms are among the plant-based foods I obtain from SHAWS and several other locations and consume frequently.

I also have benefitted and been rewarded by becoming a regular customer at THE ROSE ROOM during 2021 - consuming diverse, locally sourced and delicious meal selections prepared by JESS and BILL SABINE and their excellent staff.

As a consequence, my nutrition has been improved: better choices were made both at home, at selected local business establishments.

Avoiding heavily processed BIG FOOD food products as described by Michael Moss in his book "HOOKED" - the subject of my October post - has been a priority.

It has been relatively easy for me these past six months to forgo processed baked foods, pastry, snacks previously eaten in small amounts during past years given what I know now about their adverse coronary health risks.
